Kentucky PDHD in Phase 1a of vaccination plan

The Purchase District Health Department is currently in Phase 1a of the vaccination plan, to vaccinate those who fall into Phase 1a which includes health care workers and first responders at this time.

When Phase 1b begins, it will be released to the press.

Phase 1b includes persons 70 years of age and older and frontline essential workers.
Kentucky residents who are an interested healthcare worker with direct patient care must provide the following information to covid19@purchasehealth.org. Then notification will be made as soon as possible with an appointment time.

Information required includes: Name, Business affiliation, Direct patient care role, Date of birth, Phone number, Email, Preferred County (Ballard, Carlisle, Fulton, Hickman, McCracken), Preferred Day: Monday, Tuesday, Wednesday, Thursday, or Friday, Preferred time: 7:30 - 11:30 a. . or 12:30 - 4:30 p.m.

Those who are unable to be present 28 days from initial dose are asked not to register for the vaccine.
Residents are also asked not to call the health department to schedule a vaccine.
