The Purchase District Health Department is currently providing appointment only vaccination by phase at clinic locations and working with area hospitals to vaccinate.

PDHD is starting to schedule all Phase 1 (a, b, c) individuals for the COVID-19 Vaccination.

Availability of vaccination for the area is determined by the state as well as the order in which the vaccination can be administered.

PDHD is working directly with the state to get the vaccination to the population.

Vaccination registration form for all Phase 1 population (1a, 1b, 1c) is available at www.purchasehealth.org. An email will be sent to the email address provided to schedule online for a vaccination slot based on your phase. Scheduling subsequent phases will begin once proceeding phase is nearing completion.

For individuals that can not register electronically call 270-444-9625 x161 and leave name, number, and vaccination phase. Refer to phase descriptions below.

A phone call will be received from PDHD to schedule appointment when the proceeding phase is nearing completion.

Those who have already called to be placed on a list for when scheduling becomes available will receive a

phone call from PDHD to schedule appointment when the proceeding phase is nearing completion.

Vaccination appointments typically take 15 minutes.

Registration for Phase 2, 3, 4 is not yet available. Once this becomes available it will be publicized.

*The original Phase Description provided by the state included first responders in 1A. PDHD began vaccinations prior to the updated phases provided by the state and so has vaccinated first responders in the 1a population.

After Phase 1 is nearing completion, phase 2, 3, and 4 will proceed. Each phase will begin when

the preceeding phase is nearing completion.

Registration:

Internet based pre-registration for vaccination for Phase 1 (1a, 1b, 1c) will be available beginning

January 15, 2021.

Phase 1a Population (Healthcare workers and First Responders)

85% of 1a population will be completed by January 22, 2021 through partnerships between Purchase

District Health Department, Baptist Health, and Mercy Health.

Phase 1b population (School personnel and 70 plus population)

School population:

70% of all school personnel in our 5 counties will be completed by January 22nd, 2021.

Remaining 30% of all school personnel in our 5 counties will be completed by January 29th, 2021.

70 plus population:

70 plus population estimated to start, if vaccine is available, by Feb 1st, 2021.
